1. Photo of your VALID ID (all guests staying in the room should submit)
2. Deposit Slip / Online Bank Transaction (Transfer) / Proof of Payment
3. Screenshot of Accomplished Health Declaration Form
4. If bringing a pet (for deluxe rooms), the signed agreement form
